home health services to patients that require nursing care after
they are discharged from the hospital. Services provided are: skilled
nursing, speech therapy, occupational therapy, medical social worker,
psychiatric nursing, infusion therapy nursing, obstetric nursing,
pediatric nursing, and hospice. Our services are available 24 hours

Lifeline is an easy to use personal
response service that ensures that older adults living at home get
quick assistance whenever it is needed - 24 hrs a day, 365 days
a year. It is waterproof. It comes in a necklace or wrist band.
You just push the help button and within seconds, a certified lifeline
monitor responds, assesses the situation and summons appropriate
help, whether it is a neighbor, relative or ambulance. This service

Personal Care Services: provides
home care aide to patients who do not necessarily require the skills
of a nurse but requires assistance with personal care. Services
are available for Medicaid eligible recipients or self-pay. Call

Hospice Services provide symptom directed
care for persons in the last phases of a terminal illness so that
they may live as fully and comfortably as possible. Home health
aids for personal care, spiritual support, and trained volunteers
are also provided. Nursing services include expert pain and symptom
management, home visits, 24-hour on-call, and family assistance
counseling and social work staff to offer emotional support, counseling
